Annual collection of physician practice data. Location of practice, hours (broken down into each activity – face to face, education/teaching, admin, etc), percentage of types of patients (Medicaid/Medicare/Homeless/Sliding Fee/etc), accepting new patients, patient wait times, etc. Physical questionnaires are mailed, online questionnaires are emailed, and then follow-up calls are made.
