Long-Term Care

Long-term care refers to the services that support activities of daily living among individuals who can no longer conduct the tasks of everyday life, such as bathing, dressing, and eating, on their own. These individuals typically are older and have multiple chronic medical conditions, requiring increasingly collaborative treatment methods involving many practitioners. Treatment locations can include individualsā€™ own homes or assisted living and skilled nursing facilities.
